Background:<\/strong> Up to 10% of cases with HPT are associated with inherited syndromes, as MEN1 (HPT \/MEN1). HPT has been described in 80-100% of MEN1 cases, usually presenting as a mild disease. It is currently accepted that parathyroidectomy (PTx) improves bone mineral density (BMD) in cases with sporadic HPT (1). However, data on the bone mineral status in MEN1 and the effect of PTx in BMD of MEN1 patients have been rarely reported (2). <\/p>\n

Objective:<\/strong> To assess BMD using DEXA (proximal third of distal radius, femoral neck, and lumbar spine) in HPT\/MEN1 cases before and after PTx. <\/p>\n

Patients and methods:<\/strong> We studied clinically 40 cases with HPT\/MEN1 (20 males, 20 females; mean age 38.2±15.1 years). BMD has been evaluated in 35 out of the 40 cases. In the ongoing study, 24 cases have been already submitted to PTx and 14 of them were operated on at least 18 months ago. <\/p>\n

Results:<\/strong> Mean intact PTH levels were 116.9±88.1 pg\/dL (SD; normal values, 11-62 pg\/dL). Mean serum total calcium was 11.0±0.2 mg\/dL (NV, 8.6-1 0.2 mg\/dL) and calciuria was 329.2± 138.8 mg124h (NV, 100-320 mgI24h). Osteopenia was mostly present at the femoral neck (19\/35, 54%), but distal radius (14\/33,4204%) and lumbar spine (11\/35, 3104%) were also represented. Marked osteoporosis (T, < -2.5 SD) was seen at the lumbar spine (14\/35,40%), distal radius (13\/33, 3904%), and femoral neck (7\/35, 20%). Taken together, decreased T score values (T, < -1.0 SD) were mostly seen in the 1\/3 distal radius (27\/33 cases, 82%), but also in femoral neck (26\/35, 74%) and lumbar spine (25\/35, 71 %).In the 14 cases, BMD (g\/cm2) at the lumbar spine has significantly increased 18 months after total PTx (+7.8%; p=O.OOOl) andT-scores increased similarly at this bone site. Femoral neck BMD (g\/cm2) also increased after surgery (+7.7%; p=O.OOOl). However, no significant BMD increment was noticed at distal radius after PTx (p=OA5). <\/p>\n

Conclusion:<\/strong> Significant increases in femoral neck and lumbar spine BMDs were noticed 18 months after PTx in these 14 HPT\/MEN1 cases. This BMD pattern of response to PTx is similar to that seen in sporadic primary HPT patients.
