Furthermore data on the prevalence and features of MEN 1 among large series of NET still lack.

Aim:To evaluate epidemiology and clinical diagnostic features of MEN1 in a large multi-centric national NET study. Patients & Methods: Data regarding 1203 NET patients actively followed in 13 Italian referral centres from 1990 to 2007 were collected using the NET MANAGEMENT Software (Ibis Informatica, Milan) developed specifically for the study.

Results: Among 1023 NET pts: 242 (33%) were thoracic NET (T-NET), 476 (64%) were gastro-entero-pancreatic NET (GEP-NET). MEN1 syndrome was associated in 8.9% of patients (13.4% GEP-NET; 1.6% T-NET). A functioning syndrome was observed in 28.7% of sporadic NET and in 61.7% of MEN1 associated NET. The diagnosis was fortuity in 39% and related totumour burden in 37% in sporadic NET while in MEN1 the diagnosis was more frequently related to functional syndrome or genetic screening (85%). An overall slight female prevalence (F 55%, M 45%) was observed in MEN1 as a balance of a female prevalence in GEPNET (61% F; 39%M) and a male prevalence in T-NET (37% F; 63%M). Mean age at diagnosis was 61±16 y in sporadic and 47±16 y in MENl. Metastatic diffusion was present in 32% of sporadic NETand 23% of MEN1 NET. Overall survival from diagnosis was 2313±2070 days in MEN1 NET and 1524± 1400 days in sporadic NET.

Conclusions: The prevalence of MEN 1 in NET account for 8.9 % and is 10 fold more frequent in GEP-NET. Diagnosis is anticipated of more than 10 years in MEN 1 NETwhen compared to sporadic. They are however less frequently metastatic and associated with an higher overall survival. Different sex distribution is observed between GEP-NET and TNET in MEN1. Functional syndromes are much more frequent in MEN1 patient than in sporadic NET. Fortuity and tumour burden symptoms represent the commonest diagnostic circumstances in NET but in MEN1 they are mostly revealed by functional syndrome and genetic analysis.

Ferolla P., Faggiano A., Grimaldi F., Valcavi R., Manzoni M., Davi M.V., De Marinis L., Giuffrida D., Ferone D., Papini E., Arnaldi G., Fanciulli G., Tomassetti P., Colao A., On benealth of NET MANAGEMENT GROUP Background: Data on neuroendocrine tumors (NET) have been rarely obtained from large studies, apart from Register database.
