Age, gender, Graves orbitopathy (GO) attendance, delay before radioiodine, number of recurrences, previous treatments, corticosteroid therapy during radioiodine treatment because of GO, thyroid mass and 131I doses were analyzed. FT4, TSH, thyroperoxydase (TPOAb), thyroglobulin (TGAb), and thyrotropin receptor antibodies (TRAb) titers were measured before and 3 months after 131I. When precocious hypothyroidism occured, TRAb blocking (TBAb) or stimulating (TSAb) activity with cAMP measurement on JP cellular lines which surexpress thyrotropin receptor were determined and 123I uptake was evaluated. <br \/><strong>Results: <\/strong>Precocious hypothyroidism occurred in 23 patients (63%, group A), while 2 patients became euthyroid (6.5%) and 11 stayed hyperthyroid (30.5%) (group B). Initial TGAb titers were higher in group A (p=0.0024) and corticosteroids for an important GO were more frequently used in group B (p=0.0276). Three months after treatment, TPOAb and TGAb enhanced significantly only in group A (p=0.0112 and p=0202), suggesting important tissue damages. No iodide organification impairment was disclosed by perchlorate test. TRAb concentrations were also higher in group A 3 months after treatment, but not significantly. In this group, TBAb were present in 13 cases when hypothyroidism occurred. A transient hypothyroidism was observed in 1 patient, and it regressed concurrently to TBAb levels disappearance. <br \/><strong>Conclusions:<\/strong> Radio-induced thyroiditis appears as the main mechanism involved in precocious hypothyroidism pathogenesis after radioiodine for GD. Higher TGAb levels before treatment are associated with precocious hypothyroidism, suggesting their prognostic value. An important GO justifying corticosteroid prevention is only found in patients who relapsed hyperthyroidism.
